The drive from Schiphol to the terminal takes roughly an hour depending on traffic on the A13 and A20. Your driver checks your flight number before you land, so if the plane is delayed the pickup simply moves with it and nobody is left waiting at an empty arrivals hall. Once you clear customs your driver meets you inside the terminal, takes your luggage and drives you directly to the ship.
Plenty of cruise passengers spend a few nights in Amsterdam before their sailing, and getting from a hotel near the canals to the port on embarkation morning is one of the most requested rides we do. It takes about an hour by road, and your driver picks you up right at the hotel entrance rather than a shared meeting point somewhere in the city.

The seat of Dutch government and Scheveningen beach make for a relaxed half day out once you are off the ship, and it is close enough that it does not eat into a travel day.

Known for its car free canals and thatched roof cottages, Giethoorn is further out but a favorite for guests who have a full free day between disembarking and their onward travel.

Yes, in both directions. Pickup times are adjusted around your actual arrival or disembarkation, not a scheduled one that may not match reality.
Per vehicle. The fare covers the whole car, van or limousine regardless of how many passengers are in your group up to its seating limit.
Same day bookings are often possible depending on driver availability, but booking your disembarkation transfer a day or two ahead is recommended during peak cruise season so a driver is guaranteed to be waiting.
Child and booster seats can be added when booking, at extra charge, as long as they are requested in advance.
